Professional Issues In working on this project, we realised how to write an admission note the data and evidence from nursing school and staff as well as anecdotal information collected from other sources did not match up with some of the current approaches to nursing.
The approaches documented and used in developing material for this workbook are based on the A-G Nursing procedure, which has been replaced by other procedures in the world of nursing today. We were told about this discrepancy and also observed it when we looked at our data and some of the current textbooks on nursing.
At this point, we had a choice of either continuing with the A-G procedure or adopting a current approach. If the context data tells us something, we need to follow it. If there are other better approaches available; then these need to taken up by practicing nurses and nursing educators.

Nursing documentation is of great importance because it is one of the main tools used by healthcare providers to communicate and record specific information about patients. Nursing documentation, of which nursing notes is one type, record patient condition, any treatment given, as well as additional relevant information and thus are essential documents that help medical professionals provide better care to patients.
Inaccurate or inadequate nursing notes can lead to serious issues for the patients as well as healthcare providers. The next day the patient complained of cramping in left leg and pain in left heel. Physician ordered a heating pad applied to the affected leg. The next day the leg appeared swollen and the patient experienced pain when trying to get out of bed.
The RN also got a positive response and reported it to the manager. The RN informed the physician who advised the patient to go to therapy. Later that afternoon the patient died. She was allergic to sulfa drugs, however, a fact noted in her written record but not on the computer-generated record the resident reviewed before prescribing the drug, how to write an admission note.
